You must be readmitted and reinstated to the University. In order to do this you must:
The Undergraduate Application for Admission may be obtained from the Office of Admissions Web site at www.IllinoisState.edu/apply or www.IllinoisState.edu/welcome2isu, or by calling 800-366-2478. As a former Illinois State student, you will not need to pay the application fee.
Complete and submit an Undergraduate Application for Admission and select “Readmission” in the “Admission Category” section on page A1. In addition to the admission application, you must also include a concise, one-page, Petition for Reinstatement. You can submit your petition online. The Petition for Reinstatement must outline your thoughtful plan to earn higher grades. In addition, you may attach relevant documentation which accounts for previous failure to achieve good academic standing. Your petition and all supporting documents will be reviewed by the Reinstatement Committee.
If you have attended another institution of higher education since your dismissal from Illinois State, you must indicate such attendance on the readmission application and supply an official transcript from the other school(s). You must have earned a C average (2.00/4.00) or higher for all course work taken since last attending Illinois State and have been in good academic standing at the last institution attended as a full- or part-time student.
